Physical packets can be downloaded here. Don’t waste time in the doctor’s office! This is fast and all donations go to the sports medicine departments both schools! This supports the people who take care of your son/daughter if you they get injured!

 

RRPS Athletic Permission Forms

You will need to print, then complete and return to the school in order for your child to participate in athletics in grades 7 to 12. Be sure you have the Adobe © Reader to open these forms (download here if necessary).

Athletic Physical Packet (print, complete, submit) Required for all athletic participation!
PRE/POST Event Parent Transportation Request: To request permission to transport your child to/from athletic events

Check with the Athletic Department secretaries at your child's school if you have any questions or concerns. Be sure to visit the RRPS Health Services page to gather more information about keeping your child healthy while participating in athletics! One form you may need is:
