  Homebound Instruction Policy 410
 

Homebound instruction is available for students. These services are open to any student who, because of handicapping illness or accident, is unable to attend school for a period of 10 School Days or more, and who, because of physical, mental health or impact from a disability, cannot attend or study in group situations. Pupils considered for this program should be referred by the principal to the counselor, school nurse, special education department, or 504 Coordinator, as appropriate. As soon as an extended absence is known or expected, the school will initiate appropriate homebound services provided that the parent provides a written statement from the student's licensed health care provider including a prognosis as to the length of the student's absence and the student's ability to complete school work. Placement into a homebound program for a student with a disability must have prior recommendation by the Individualized Educational Program, or 504 Committee, as appropriate. Reentry into the school setting shall be contingent on approval by the physician, the Individualized Educational Program or 504 Committee. For the duration of the home bound services the student shall be considered in school attendance provided that the services have been appropriately provided by the school according to the General Education Homebound Plan, Individualized Education Program or 504 Accommodation Plan.
